History Form

Here is the form that I have my clients fill out just one time. The first time you come in for a massage you fill this one out, it gives me the necessary information to know what kind of massage you need. It also has all my legal information as well. If you print this form and you can't read the bottom portion, please come in for a clear copy or shoot me and e-mail and I'll send you the PDF version.

If you have scheduled a massage with me, you can print this form (right click/save picture/find file/print), just to be ahead of the program on the day of your visit. Otherwise I have them readily available when you come in.
